ABSTRACT

COVID-19 has a significant impact on the hematopoietic system and hemostasis. Leukocytosis, lymphopenia, and thrombocytopenia are associated with increased severity and even death in COVID-19 cases. Objective: The aim is to examine the laboratory results of COVID-19 patients from a hospital in the Peruvian Amazon and their clinical prognosis. Material and Methods: An analytical cross-sectional study was carried out whose purpose was to identify the laboratory tests of patients with COVID-19 and mortality in a hospital in Ucayali, Peru during the period from March 13 to May 9, 2020, selecting a total of 127 with Covid-19. Mean and the standard deviation was described for age, leukocytes, neutrophils, platelets, RDW-SD;median and interquartile range for the variables lymphocyte, RN / L, fibrinogen, CRP, D-dimer, DHL, hematocrit, monocytes, eosinophils. Results: No differences were observed in this population regarding death and sex (OR: 1.31;95% CI 0.92 to 1.87), however, it was observed that, for each one-year increase, the probability of death increased by 4% (PR: 1.04, 95% CI 1.03 to 1.05). The IRR (Incidence Risk Ratio) analysis for the numerical variables showed results strongly associated with hematological values such as Leukocytes (scaled by 2500 units) ( IRR: 1.08, 95% CI 1.03 to 1.13), neutrophils (scaled by 2500 units) (IRR: 1.08;95% CI 1.03 to 1.13), on the contrary, it is observed that the increase of 1000 units in lymphocytes, the probability of dying decreased by 48% (IRR: 0.52;95% CI 0.38 to 071). Conclusions: Parameters such as leukocytes, neutrophils and D-dimer were statistically much higher in patients who died.

ABSTRACT

Background: Neoplastic pericardial effusion (NPE) is a serious complication that occurs in the setting of advanced oncological disease and is associated with a high recurrence rate. Currently, pericardiocentesis (PCT) remains the first therapeutic option and the use of percutaneous balloon pericardiotomy (PBP) is limited to the treatment of recurrences. However, it is not known whether some aspects of the procedure during PBP lead to different outcomes in terms of survival and recurrence, and no such patients have been included in studies during COVID-19 pandemic. Purpose(s): The aim is to analyses the success, complications and recurrence rate (defined as recurrence of NPE requiring PCT, PBP or surgical pericardial window (SPW) of both procedures (BP) in order to establish the optimal entry treatment for these patients. Method(s): This research analyzed the clinical characteristics and prognostic factors of patients with severe pericardial effusion of neoplastic etiology who underwent PBP during the COVID-19 pandemic. A prospective study was conducted involving 23 patients admitted between January 2020 and January 2022 for severe NPE who underwent PCT or PBP as initial treatment of NPE. Result(s): We included 23 patients, 62.9% were male with a mean age of 51.2+/-14.9 years NPE was the first manifestation of the oncological process in 12 patients (52.1%) with lung cancer being the most frequently associated primary cancer (58.7%) followed by breast cancer in 12.7% of cases. A total of 26 procedures were performed, 10 PCT, 15 PBP, 1 SPW, with tumors cells identified in the pericardial fluid in 13.0% of cases. PCT was used as an entry point in 10 patients (43.5%), 6 patients were COVID-19 positive and PCT was performed as the first treatment. While PBP was chosen as the first therapeutic option in 13 patients (56.5%) (2 Re-PBP). The initial efficacy of the procedure was 93.1% and 92.2% respectively (p=0.88), with 1 complication occurring in the PBP group but not requiring scheduled SPW. In the former group, the percentage of recurrences was higher (34.7%;8 recurrences in 10 patients) compared to patients treated with upfront PBP (8.6%;2 recurrences in 13 patients), p=0.09. In addition, only one patient had to resort to surgery. When analyses according to the BP. used, the recurrence rate was 4.0 times higher for PCT (34.7 vs. 8.6% recurrences), although without reaching statistical significance (p=0.16). Conclusion(s): The PBP is a simple, safe and effective technique for the treatment of NPE during the COVID-19 pandemic, in our series it was associated with a lower recurrence rate. Therefore, it could replace PCT in these patients during the COVID-19 pandemic as optimal first line treatment, providing better quality of life and reducing the need for re-interventions. ABSTRACT

This paper develops an analysis of the deep production contraction of Mexico's 32 states in the second quarter of 2020 as a consequence of the COVID-19 pandemic. Cross-section econometric models are estimated (in the absence of spatial effects);the main results suggest that variables associated to the international context, such as remittances, degree of international integration and basic tradeable goods production do not have statistically significant effects on the drop of production, while manufacturing production, particularly that of durable goods, and touristic and leisure activities are significant in explaining it. © 2022 Universidad Nacional Autónoma de México, Facultad de Economía.

ABSTRACT

This review paper analyzes the state of knowledge on Telepsychiatry (TP) after the crisis caused by COVID and the resulting need to use new modalities of care. Six essential aspects of TP are addressed: patient's and mental health staff satisfaction, diagnostic reliability, effectiveness of TP interventions, cost-effectiveness in terms of opportunity cost (or efficiency), legal aspects inherent to confidentiality and privacy in particular and the attitude of professionals toward TP. Satisfaction with TP is acceptable among both patients and professionals, the latter being the most reluctant. Diagnostic reliability has been demonstrated, but requires further studies to confirm this reliability in different diagnoses and healthcare settings. The efficacy of TP treatments is not inferior to face-to-face care, as has been proven in specific psychotherapies. Finally, it should be noted that the attitude of the psychiatrist is the most decisive element that limits or facilitates the implementation of TP. Copyright © 2022 The Author(s)

ABSTRACT

Introduction: The use of ivermectin in the treatment or prevention against COVID-19 poses a public health problem. Objective: Describe the inappropriate use of ivermectin for the treatment and prevention of COVID-19. Methods: Prospective cross-sectional study. A snowball-type sampler was used. All those who agreed to participate in the study (163) and who agreed to participate voluntarily were interviewed. The participants belonged to the departments of Áncash, Apurímac, Arequipa, Ayacucho, Cajamarca, Callao, Cusco, Huancavelica, Huánuco, Ica, Junín, La Libertad, Lambayeque, Lima, Pasco, Piura, Puno, Tacna and Ucayali (Peru). The study was conducted during the month of May, 2021. Results: 163 participants were surveyed, of which 71 (44%) were men and 92 (56%) women, with a mean age of 56.41 (SD 10.24) and with a minimum of 21 and a maximum of 82 years. The most relevant was the frequent use of ivermectin for the spread of COVID-19, although it was consumed more time by people who used it as prevention method. Conclusions: People consumed ivermectin once or twice a day, without a prescription, as prevention or because they had COVID-19. Those who took it as a preventive method took high doses for a long time putting their health at risk. This situation could be reversed through the proper implementation of science-based directives and a better media information policy.

ABSTRACT

Worldwide concern about the novel coronavirus (2019-nCoV) as a global threat to public health is the reason for the exponential growth of phylogenetic analyses. The purpose of this review was to describe the mode of operation and advantages of the tool Nextstrain, as well as the sequencing of the SARS-CoV-2 virus worldwide. The interface of the Nextstrain page was used to show its functions and data visualization modes. These were downloaded from the website GISAID to show the number of SARS-CoV-2 sequencing processes performed so far. Nextstrain is an open code project created by bioinformatics biologists to make good use of the scientific and public health potential of data about genomes of pathogens. Nextstrain consists in a set of tools operating with unprocessed sequences (in FASTA format). Nextstrain performs a sequence alignment of the input data into a multiple sequence alignment based on fast Fourier transform. Its use is based on two software applications: Augur and Auspice. Nextstrain is an efficient tool by which lay people may obtain epidemiological data in a simple manner. It may be used in the public health sector, since it shows real time data about epidemics and their geographic distribution. It may also be used to follow-up outbreaks, as is the case with COVID-19. © 2021, Centro Nacional de Informacion de Ciencias Medicas. All rights reserved.

ABSTRACT

BACKGROUND: Sustained fear during pregnancy has the potential to increase psychological distress and obstetric risk. This study aimed to (1) identify factors and characteristics associated with fear of COVID-19, (2) investigate the relationship between fear of COVID-19 and maternal anxiety and depression, and (3) determine the relationship between fear of COVID-19 and pregnancy outcomes. METHODS: 9251 pregnant Canadians were recruited between April - December 2020. Participants self-reported (scale of 0-100) the degree of threat they perceived from the SARS-CoV-2 virus in relation to themselves and their unborn baby. RESULTS: Mean fear scores indicated moderate to elevated concern. In multivariable linear regression, fear of COVID-19 was associated with food insecurity, ethnicity, geographic location, history of anxiety prior to pregnancy, having a chronic health condition, pre-pregnancy BMI, parity, and stage of pregnancy at study enrollment. Higher COVID-19 fear was associated with increased odds of depression, adjusted odds ratio (aOR) = 1.75, p < 0.001, 95% CI 1.66-1.85, and anxiety, aOR=2.04, p < 0.001, 95% CI 1.94-2.15). Furthermore, fear of COVID-19 was associated with a 192-gram reduction in infant birthweight, and a 6.1-day reduction in gestational age at birth. LIMITATIONS: The sample has higher education compared to the Canadian population and cannot test causal effects. CONCLUSION: This study suggests that sociodemographic, health, and obstetric factors may contribute to increased fear of COVID-19 and associated adverse psychological and pregnancy outcomes.

ABSTRACT

This article describes how Mexican students and teachers in High School are adapting to COVID-19 emergency situation. For this aim were collected in a sample of n=11,850 teachers and n=39,852 students that represent different geographical regions and public and private institutions. An online questionnaire was designed to study three dimensions of this social phenomenon: The pedagogical, psychological and, the technological. Among the main findings are the following: Pedagogically, teachers and students have been flexible, looking for ways to adapt teaching and learning, combining digital and traditional resources. Technologically, only half of teachers report adequate Internet connectivity, which limits their use of digital resources. Psychologically, discouragement is identified, as well as vulnerable social-emotional individuals. Through regression tree analysis, the importance of learning assessment and contact with students is revealed, both for learning and for social-emotional well-being. The research is relevant because it allows us to see strengths and areas of opportunity for the Mexican Educational System.

ABSTRACT

The COVID-19 pandemic has brought changes in the working conditions of workers, these effects are appreciated from the income to their health, considering this situation;the purpose of this research was to investigate the effect of teamwork and workloads on burnout in workers under the context of the SARS-CoV-2 virus pandemic. Data were collected through an instrument in a sample of 350 workers from eight medium-sized companies established in the metropolitan area of Mexico City. The hypotheses were tested through a structural equation model using the AMOS 20 software. The results indicate that, under the circumstances of a pandemic, the organizational climate and workloads have interrelated behaviors; However, these variables are negatively related to burnout, this means that the variations that may suffer as a result of the pandemic do not have significant effects on worker burnout;therefore, burnout will have its origin in other circumstantial factors. © 2020, Universidad del Zulia. ABSTRACT

The aim of this paper is to analyze the effect of a large battery of demographic, social, health and economic factors on the magnitude and intensity of SARS-CoV-2 contagion in the Mexican states. To do so, an extreme-bounds analysis in cross-section econometric models, with possible spatial dependence, is carried out. Our findings suggest that a greater population density (that impedes social distancing), the suffering of obesity and/or chronic degenerative diseases (diabetes and hypertension) and the lack of respect for health regulations have favored the spread of COVID-19. Social conditions of population and economic characteristics seem to be not relevant. The public policy implications from our results are straightforward. © 2020 Universidad Nacional Autonoma de Mexico. All rights reserved.
